(If you have been uninsured for six months and are interested in applying for GettingUSCovered, please click here. All CoverColorado applicants who have been uninsured for at least six months will have their applications pended to notify them of the GettingUSCovered option.
If you submit an online application, you must print it out after completion. Your application must include a hand written signature and date. The hard copy application and supporting documents must be sent to CoverColorado's enrollment office via: email/scan, fax, delivery, or mail. You may scan and email your application and documents to: email@example.com, mail them, fax to: 303-962-1145. If you mail your application, we recommend sending it via certified, return receipt or overnight, so you have proof of delivery. Our mailing address is:

Completing an online application, without printing and: faxing, emailing/scanning, delivering or mailing it with a hand written signature and date, do NOT meet our deadline. Online signatures are NOT acceptable. The Plan Summary and Rates are for your file and do NOT need to be submitted with your application. If your adjustable household income and your liquid assets are less than $50,000, you may be eligible for the premium discount. You must submit a separate Premium Discount Application.
A complete application must be received by CoverColorado by the 15th of each month to be eligible for a 1st of the following month effective date. If the 15th falls on a weekend, the application must be in our office the Friday before. Please note that it is taking 3-4 weeks to approve applications. Applications are processed in the order they are received, so applications received on or just prior to deadline may not be approved until after the first of the following month. As long as you
emailed/faxed a complete application with all supporting documentation, we will
honor your requested effective date.
Due to the high volume of applications received in our office, we are unable to confirm receipt. If you fax the application, please keep a copy of your fax confirmation page; if you email an application, please be sure to keep a copy of the email that's in your Sent folder. If you mail the application, we highly recommend that you mail it via Return Receipt or Certified mail, so you have confirmation of receipt.
